you were killed by FRANK THA TANK
do +scores in console then do timescale -1 at 4:40
from the start it never said FRANK THA TANK killed someone. for example after 4:40 if you notice carefully FRANK THA TANK had 4/3 then as soon as you were killed his score went up 5/3, so it was FRANK THA TANK who killed you
only thing i cant answer is why it doesn't say his name after "killed by"

he wasn't invisible, first time you were killed he was at podium, second time he just naded from kitchen door. use thirdperson views (cg_third)if you want to see if he really was invisible or not
